Our Medical Home functions as a primary care office but is located at Children’s Medical Center. This service is offered for those physicians who care for patients with complex care needs but feel they could be better managed at Children’s Health℠ due to  ready access to specialists, comprehensive records and full ancillary support.

Our Medical Home offers a dedicated provider who can span the continuum of outpatient and inpatient care for patients, helping to provide clinical background and ensuring consistency in care. Other benefits of the Medical Home include around-the-clock phone availability, access to same-day sick visits, health and wellness services, chronic condition management, and medication management and reconciliation.
